About Us
The Pennsylvania Care Foundation provides home care services for the elderly and the infirmed assisting people with their medical care as well as their personal home care needs.
The Pennsylvania Care Foundation provides a recipient/participant centered approach by adhering to the personal care plan determined by the corresponding MCO, medical care organizations of Pennsylvania and/or the specific needs of the participant and his/her caregivers. Our caregivers are professionally trained to provide all necessary care services as determined by the participants needs & requirements. All participant centered services include:
- Assisting with the necessary precautions & recovery during times of illness & injury
- Preparing healthy and nutritious meals.
- Cleaning & sanitation of bathrooms, living areas, kitchen and bedroom space.
- Cleaning and preparing laundry.
- Help with activities of daily living, bathing, personal hygiene and personal care routines (Dressing & Personal Grooming).
- Help with grocery shopping, errands & community-based activities.
- Arranging medical appointments, ensuring medication intake and arranging for transportation for the participant.
- Provide companionship, reading, cognitive activities, conversation, media & play games.
- Promote healthy habits and exercise routines that support the participants health & well-being.
- Sustaining good communication with family members to maintain contact & reassurance for their loved one.
